It’s Easy to Get Started
If you’re a new patient, please arrive a little early to your first appointment and bring a photo ID and your insurance information. We'll ask about your health and dental history, any medications you're taking, and some other general questions on our new patient form. If you prefer, you can fill out this paperwork ahead of time by downloading it here.
We take care of filing claims with your insurance to make sure your dental care is as affordable as possible. At the time of your dental service, you can pay by cash, check, or credit card. Or, ask about our payment financing options.
